Our Programs

Love Community Action Center's Senior Support Program offers comprehensive care coordination and social engagement services to seniors within our communities. Our team provides essential services such as home visits, hospital visits, appointment reminders, and medication adherence, as well as recreational activities, mentorship, and advocacy.

SENIOR SUPPORT PROGRAM

Supporting Seniors with Love Community Action Center
At Love Community Action Center, we understand the importance of providing seniors with the support they need to live healthy and fulfilling lives. That's why we've created our Senior Support Program, which offers a range of services specifically tailored to meet the needs of our older members.

Our program includes care coordination, engagement activities, an internship program, a mentorship program, and advocacy for the rights of all citizens. With our Senior Support Program, we're committed to helping seniors in our community stay healthy and connected.


  • Engagement- We will work to ensure our aged population are engaged, connected social day programs and recreational services for therapy, weekly entertainment, trips etc.. 


 

THE YOUTH

RE-INTEGRATION PROGRAM

This will be a robust program that helps juvenile delinquents who we were incarcerated to be fully integrated back into society within a year of being released from jail and being under probation or supervised parole. This will significantly reduce the number of young individuals being reincarnated or becoming a menace to society. This will see a decline in domestic violence, hate crimes, petty crimes and lawless behavior. 

 

In this framework we will identify and collaborate with the case workers at juvenile and detention centers and where new cases will come to our center for enrollment based on the case workers recommendation. 

 

We can also work with those who are at risk of becoming juveniles by recommendation of the courts or dept of justice. This will be to re direct identified individuals from their current path to a new path of prosperity via new established pathways of growth. 

 

This would see us building close relationships with community business partners, neighborhood educational centers and stake holders in order to build trust and bridge the gap  of disconnect between juveniles and their place in society. 

 

This program will include a road map to stability for getting education, working on clearing overcoming (criminal history related) employment barriers or developing entrepreneurial skills to thrive in the workforce todays emerging economies needs.


CASE MANAGEMENT PROGRAM


Our comprehensive case management program will seek to identify and address existing gaps in care whilst reducing transitions to care. 


Within this framework our care team will provide:

Home visits (wellness checks), visiting our shut in members, hospital visits, schedule appointments, do appointment escorts, schedule medical transportation, making appointment reminders, setting up medication delivery, providing education on insurance benefits/insurance navigation. We assist families with obtaining child care vouchers and other programs funded by department of youth & community development and the department of aging like SCRIE & DRIE. We will also assist eligible persons with applications for public benefits through HRA along with the annual recertification process. 


HOUSING SUPPORT

The Love Community Action Center works to end the perpetual state of instability caused by chronic homelessness. 

We do this by providing housing support where we identify suitable housing resources and make the linkage to :

  • Supportive Housing- (congregate or scattered site)
  • Assisted living
  • Transitional living 
  • Independent living.


We assist eligible persons with obtaining housing vouchers. 

We also assist with prevention court evictions by connecting individuals to resources to address rental arrears. 


MENTORSHIP PROGRAM

Each member will be placed on a team with a schedule of weekly activities. It will be a mixture of both youth and senior members. Seniors will be utilized for sharing and educating on selected topics, while the youth will be used to assist seniors with developing their computer literacy, finance management and proficiency with using their phones, social media etc.. this care model is deliberately designed to build community relationships, trust, love and a sense of unity.

ADVOCACY  

As an organization we plan to advocate for the basic human rights of every citizen within the communities we serve. We challenge any attempts to dismantle any laws that are critical to the continued care and support for our clients. We also utilize our collective voice at community boards and in political spaces to champion for the rights of those who don’t have a platform.
